xen: use header for EXPORT_SYMBOL_GPL

mmu.c needs to #include module.h to prevent these warnings:

 arch/x86/xen/mmu.c:239: warning: type defaults to 'int' in declaration of 'EXPORT_SYMBOL_GPL'
