Pull USB fixes from Greg KH:
 "Here are some USB fixes for 4.5-rc3.

  The usual, xhci fixes for reported issues, combined with some small
  gadget driver fixes, and a MAINTAINERS file update.  All have been in
  linux-next with no reported issues"
